Registration for 2009-2010
Registration packets for new entrants are available at the Momot, Bailey Ave. or Oak St. Elementary Schools & Stafford Middle School Offices.
The completed enrollment packet must be returned to the main office at Momot, Bailey Ave. or Oak St. Elementary Schools & Stafford Middle School and must include:
- a completed registration form
- signed permission to participate in program
- a copy of your child’s immunization and medical record
- a photograph of anyone permitted to pick up your child.
We serve children on a first come, first served basis, and some class sizes may be limited. A waiting list will be instituted once all available openings are filled.
