Please apply now! Well-fed children do better in their school work, and the District receives additional funding for each student who participates in the free or reduced price lunch program, which helps keep prices reasonable for all families.

If you think your child(ren) may qualify for free or reduced price meals, please complete and return the enclosed application for free and reduced price meals as soon as possible so that your child(ren) will not have to wait for the application to be processed in September. If you send in the application now, and you qualify for free or reduced price meals, your child will receive free or reduced price meals the first day of school.

If your child(ren) received free or reduced price meals last year, please remember that you must submit a new application again this year. Only one application per household is required.

You may mail your application to your child’s school or send it in with your child . If you have any questions concerning the application, please call your child’s school. They will personally assist you if you request.

If you receive food stamps or Aid to Dependent Children (ADC/TANF), you will receive a Direct Certification Letter from the New York State Office of Temporary and Disability Assistance this month. Send this letter to us and you will not need to complete an application for those children listed on the Direct Certification Letter. Any child in your household not listed on that letter will need an application.

APPLICATION INSTRUCTIONS SIMPLIFIED  

Question #1- List all students in the household and the grade or teacher and school they will attend this year. Separate applications for each student are not necessary, except in the case of foster children.

Question #2 - If a foster child is in the household, check the box to indicate that and show the child’s income. Skip questions 3 and 4; simply sign in section 5.

Question #3 - If your household is eligible for food stamps, print your food stamp case number. We cannot accept an “ATM” or benefit card number. If you are not sure of your case number, please call your case representative at the Department of Social Services. If you received a letter from New York State Office of Temporary and Disability Assistance indicating that your children listed on that letter are approved for free lunch and breakfast, send that letter to us. You do not need to fill out an application for the child/children listed on that letter

Question #4 - Gross income ( before tax and social security deductions) must be reported by all members of the household unless a food stamp case number is provided. Record the amount as weekly, every two weeks, two times a month, or monthly. Income includes wages, tips, income from self-employment, pensions and retirement income, unemployment compensation, public assistance payments, Supplement Security Income or Survivor’s Benefits, alimony or child support, and disability benefits, including workers’ compensation.

Question #5 - An adult household member must sign, date and record their Social Security number whenever income is listed.
