New York State Education Law requires a physical examination for all new entrants, tenth grade students, students participating in interscholastic sports, and students with special needs. Ideally, the student’s physician or health care provider should perform the physical examination. However, our district Nurse Practitioner can perform physicals in the school health office if needed. The student can stop by the health office to schedule a physical exam with her. For those students who do not provide a physical exam from their physician/health care provider or do not schedule a school physical in a timely manner, will automatically be scheduled for the mandated physical here at school.
A physical examination is required for a student to obtain working papers. The physical examination must have been performed within 12 months prior to the issuance of the certificate.
All athletes must have a yearly physical examination. The physical results are valid for a period of 12 months through the last day of the month in which the physical was conducted.
An updated health history is required yearly. Click here to download the Health History Form.
