Sports sign-ups and clearances begin 30 days prior to the first practice. Daily reminders on the school announcements will let the student know they should stop by the health office to signup and check on their clearance status.
All athletes must have a yearly physical examination. The physical results are valid for a period of 12 months through the last day of the month in which the physical exam was conducted. In addition to the physical exam a yearly Health History form must be completed and signed by the parent.
If a student has asthma or has used in inhaler in the past they must have an Inhaler order completed by their physician or a note clearly stating they no longer need the inhaler signed by their physician. This order/note must be given to the school nurse before they may begin practicing.
All injuries should be reported to the school nurse along with an injury specific note from their physician stating when they are permitted to return to sports/gym; however the school physician has the final authority to determine if the student is capable to participate. The note must go to the school nurse who will forward the information to the necessary coach, gym teacher and athletic trainer.
If the student has any history of cardiac problems and has had EKG or Echocardiogram testing done please have your doctor send a copy of the test report to the school nurse. Often, our school physician or nurse practitioner will need to review this information to determine if it is safe for the student to be cleared to participate in physical activities/sports.
Students are advised to send in a note from their doctor or copies of test results as soon as possible to avoid any delays in sports clearance.
