New York State Education Law requires that no medication shall be given without a written order from a physician or licensed prescriber and written parental permission. Only those medications which are necessary to maintain the student in school and must be given during school hours should be administered. Any student who is required to take medication during the regular school day or while participating in school-sponsored activities (e.g., field trips, athletics) should comply with the following procedures:
a. Student’s name and date of birth
b. Name of medication
c. Dosage and route of administration
d. Frequency and time of administration
e. For prn (as necessary) medications, conditions under which the medication should be administered
f. Date written
g. Prescriber’s name, title, and signature
h. Prescriber’s phone number
*Students are not permitted to carry medications on their person or keep them in their locker.All medications must be kept in the Health Office.(except inhalers *see below)
*Students may obtain a “Self Administration Order” from their physician to allow them to self carry an Inhaler.
*At the end of the school year all medications must be picked up by the parent.If they are not picked up they will be discarded.
*Each school year you must bring in a new written order from your physician or licensed prescriber.
